闌尾又像胃腸道其它部位很薄的派爾淋巴集合小結構———從腸道內容物中吸收抗原并對這些內容物產生反應。這個局部免疫系統在生物及控制食物、藥品、微生物和病毒抗原方面均起了至關重要的作用。這些局部免疫系統和腸道炎癥，以及附屬于全身免疫系統的自體免疫反應的關系，還在研究之中。由此看來，闌尾的功能似乎是使白細胞接觸胃腸道里的大量抗原，即外來物質。因此，闌尾可以幫助抑制具有潛在破壞作用的體液性抗體反應，同時能夠提供局部的免疫作用.闌尾吸收腸道內的抗原并對其作出反應.這種局部的免疫系統在生理免疫反應以及對食物，藥物，細菌或病毒性抗原的控制中發揮了重要的作用。科學家正在對這些局部免疫反應與炎癥性腸疾病以及自體免疫反應之間的關系進行研究。如果腸內細菌平衡失調，就可能導致潰瘍性大腸炎和節段性回腸炎，還容易發生食物中毒，因此不要輕易切除闌尾。
